If you wish to apply for a mandatory NHS contract or an additional (domiciliary) contract with the Area Team you will need to complete the standard national forms and carry out some pre-application checks (eg disclosure and barring, previously CRB). The Area Team will also need to carry out checks on receipt of your application and carry out a practice visit BEFORE any NHS contract is issued.
